A membership-based practice allows physicians to care for a smaller number of patients in order to provide a more personalized medical experience with enhanced access, communication, and continuity of care.

If I decide not to enroll now, can I join later?

Because we intentionally limit the number of patients each physician cares for, membership availability may become limited over time. Once a physician reaches capacity, patients requesting enrollment may be placed on a waiting list.

What is the cost of the membership?

Annual memberships fees range from $3,000 to $3,500, depending on membership availability and physician panel capacity. Membership is limited to maintain smaller patient panels, enhanced access, and personalized care. Membership fees are billed annually and support the enhanced services and benfits provided through Community physicians of Westchester.

How does insurance work with membership?
We plan to participate with most major commercial insurance plans, as well as Medicare. The membership fee is separate from your insurance and covers enhanced access and non-covered services associated with our practice model.

Because payer credentialing and enrollment are still in process, we are not yet able to provide a final list of participating insurance plans. We will continue to provide updates regarding specific insurance plans as information becomes available.

Does the membership fee count toward my insurance deductible?

Membership fees generally do not apply toward insurance deductibles. However, they may qualify as eligible HSA/FSA expenses depending on your individual plan. We recommend confirming with your benefits administrator or tax advisor.

What are the office hours?

Office hours will be announced closer to our opening date.

How can I reach the office?

Patients will be able to contact the office by phone, patient portal, and other direct communication methods provided upon enrollment.

Will I always see my primary physician?

Whenever possible, yes. One of the goals of our practice model is to strengthen continuity and foster long-term physician-patient relationships.

What if my primary physician is unavailable?

If your primary physician is unavailable due to vacation, illness, or other circumstances, another Community Physicians of Westchester physician may be available to assist with your care when appropriate. Our physicians work collaboratively to help ensure continuity of care while maintaining your established relationship with your primary physician.

Will telehealth visits be available?

Yes, telehealth appointments will be available when medically appropriate.

Do physicians provide after-hours coverage?

If you need to speak with a physician after the office is closed, calls will be directed to your physician or the on-call provider from our practice.

Please note that we are not a substitute for emergency medical services. In the event of a medical emergency, call 911 or proceed to the nearest emergency room immediately.

For routine or non-urgent matters after hours, patients may leave a voicemail or send a portal message, and we will respond as promptly as possible.

Will you have access to my prior medical records?

Yes. We will work with patients to obtain relevant prior medical records whenever possible.

Which hospitals are your physicians affiliated with?

Our physicians maintain medical staff privileges at NewYork-Presbyterian Hospital and White Plains Hospital.

While we remain an independent physician practice, we coordinate care closely with specialists and institutions throughout the New York metropolitan area to ensure patients receive the care best suited to their individual needs.
